In order to obtain any Driver's License, CDL,  State of Ohio ID Card or duplicate of any of these items, the customer (or any co-signer for a minor) must present two (2) documents satisfactory to prove  both: 1)Date of Birth and 2) Social Security Number (SSN), if ever assigned.

Only the original documents or a copy bearing an original certification by the issuing authority are acceptable. Uncertified or copies  of certified documents are not acceptable. Failure to provide two (2) acceptable  documents to prove both the Date of Birth and the Social Security Number, if ever assigned shall result in denial of the application.

***Please Note: Additional Documentation may be required if the documentation provided is questionable.

Acceptable Document List

Primary Documents- Must include Date of Birth

 

Drivers License - (Any State or Canada) with photograph, current or expired not more than six months.

 

USA Birth Certificate- Must be original or certified copy, have a seal and be issued by an appropriate government agency.

 

State Issued Identification Card- Any State with photograph, current or expired not more than six months.

 

Immigration and Naturalization Service (INS) Documents- Must be original and valid.

 

Certified Copy of Court Order (Adoption and Name change documents only!)- Must contain full name and date of birth.

 

Military Indentification- With photograph.

 

Valid Passport- U.S. or Canadian, with photograph. If from any other country, appropriate INS document is also required.

 

 State Issued Learners Permit- (Any State) with photograph, expired not more than six (6) months.

Secondary Documents- Must include SSN if not on primary document

 

Official Social Security Card or Number Indentification (Numident) Printout
Metal card not acceptable

 

Bureau of Indian Affairs Card/Indian Treaty Card -
Tribal Indentification card not acceptable

 

Certified Copy of a Court Order - With or without date of birth.

 

Credit Card

 

Employer Indentification Card - With photograph

 

Foreign Birth Certificate - Must be accompanied by a translation by an approved translator.

 

Health Insurance Card - Example: Blue Cross/ Blue Shield.

 

Internal Revenue Service or State Tax Form - Issued by the Governmental agency. W-2 not acceptable.

 

Marriage Certificate or License

 

Medical Treatment Records from a Doctor or Hospital

 

Military Dependent Indentification - With photograph

 

Military Discharge Separation Papers

 

Gun Permit - With photograph.

 

Pilot's License

 

School Record or Transcript - Must be certified copy.

 

Student Indentification Card - With photograph issued by a school or university.

 

Vehicle Title - Vehicle registration is not acceptable.

 

Voter Registration Card - Please note that Ohio does not submit such a document.

Reinstatement

Several reinstatement centers are available locally in order to convenientally suspended license reinstated.

 

Social Security Card and Birth Certificate Replacement

A replacement Social Security Card may be obtained at the following location:

Ordinary Replacement

Generally, to replace a lost, stolen or revoked license, the applicant would present a certified birth certificate and Social Security Card. If either are not available, then other documents must be presented to prove  the Date of Birth and Social Security Number, if ever assigned. Refer to the general instructions for the preceding acceptable document list.

Ordinary Renewal

Generally, to renew a driver's license or ID, a person would present the old license or ID and another document from  either the primary or secondary list.
